Case 1:

The patient came to us for a second opinion because another dentist told him he requires a root canal therapy and a crown on tooth #14.  The tooth has large existing silver filling (amalgam) and a large recurrent tooth decay underneath the filling.  The other dentist isn't wrong to suggest a root canal therapy in this case because the decay is very close to the nerve.  However, the patient couldn't afford the large treatments at the moment but still wishes to save the tooth.  The patient and we discussed some options and we suggested to remove the decay and see if we can save the tooth with a filling as a short-term solution, the patient agreed to this option.  From the images below, you will see the red circles show where the decay is from the picture taken before treatment and where the successful filling is from the picture taken after the composite (tooth-colored) filling is placed.  The patient was able to postpone the root canal therapy and was very happy with the outcome.

Case 2 -

The patient came to us for a regular exam and we found she has a large existing filling with more tooth decay underneath it.  The decay is getting very close to the nerve and the typical treatment for this condition would be root canal therapy and crown.  However, the patient was very hesitant to consider root canal therapy due to poor experience from the past.  After we discussed a few options, we suggested to remove existing filling and decay and see if the tooth can be saved with a filling as a short-term solution, the patient agreed.  From the images below, you will see the red circles show where the decay is from the picture taken before treatment and where the successful filling is from the picture taken after the composite (tooth-colored) filling is placed.  The patient was able to postpone the root canal therapy and was very happy with the outcome.

Case 3 -

The patient came to us for a regular exam and told us she has a large cavity on her wisdom tooth.  Our exam showed not only a large cavity on her wisdom tooth but also a large cavity on the tooth right next to it.  While the decayed wisdom tooth can be extracted, we would like to save the tooth next to it.  This filling is very difficult to do because the wisdom tooth is wedged right against it and the decay extended very far below the gum.  Red circle below shows the decay on the wisdom tooth and the tooth next to it before treatment.  The other red circle shows the successful filling on the tooth next to the decayed wisdom tooth after treatment.  Now the patient can go to the oral surgeon to remove the decayed wisdom tooth!
